ÄûÃʵ¼°¹ Early Support Service for young children with disabilities

The Early Support Service is for families of children aged 0 to 5 with disabilities or complex medical needs.

What does Early Support do?

We let you know about the support available to you and your family. We make referrals and applications to services you want to access.

We support you to become more confident and resilient.

Our team can visit you at home. We will keep in touch with you regularly, not only when there is a problem.

We can offer:

  • information, advice and practical support about local services
  • key worker support short-term (up to 6 months) or long-term (up to a year) depending on the needs of your child and family
  • support through groups
  • a Team Around the Family meeting for families that have children with very complex needs

How to get support

Speak to someone who is supporting your child. This can be a health visitor, therapist, local family centre or portage worker.

They know if your family can get early help support and make the referral.

They complete a  and email it to cspa@surreycc.gov.uk


What happens after a referral?

We visit you at home to discuss the level of support that is right for your family. We work out who will be involved and for how long.


Early Support referral criteria

  • Child with complex health needs, disabilities or special educational needs aged 0 to 5 years (you do not need a diagnosis)
  • Three or more specialist health or education professionals involved (or referred to)
  • Parent or carer agrees to Early Support involvement.
  • Family in need of regular input to build resilience and improve child's outcomes. Family face  significant social, financial, health or community challenges.

Email the Early Support Service for referrals for:

  • life limited children
  • babies with complex needs on discharge from hospital
  • new to area children
